Replace Washington Drivers License While Out of State

Driver License Replacement/Renewal Request

Use this form to apply for a duplicate or renewal of your standard Washington State driver license if you are outside of Washington State. The form must be completed in full and all questions must be answered.

You cannot use this form to renew or replace your enhanced driver license (EDL). You must visit a driver licensing office in person to get a duplicate or renewal of your EDL.

We will send your license to the address you provide on your application. (We don’t mail to Washington State addresses.) Your license will be labeled “identity not verified” and will not contain a photograph or signature.

Instructions

Please print this form on white paper and include all of the following with your completed application:

• All necessary fees.
• Any supporting documents required for license restrictions and endorsements as described below.

License restrictions and endorsements

• Medical/vision certification: If you are required to periodically send us medical or vision certificates, you must include the certificate with your completed application form. You can request a medical or vision certificate form by calling (360) 902-3900 or sending an email to drivers@dol.wa.gov. A competent medical authority or licensed eye care professional must complete the form.
• Motorcycle endorsement: You may renew a motorcycle endorsement along with your driver license for an additional $25 fee. If you wish to surrender your motorcycle endorsement, you must submit a Surrender form with your application. You can request a Surrender form by calling (360) 902-3900 or sending an email drivers@dol.wa.gov. Once you surrender your motorcycle endorsement, you must pass all applicable tests and pay the fees to get an endorsement again.
• Commercial driver license: If your commercial driver license (CDL) is lost or stolen, you may request a duplicate by mail. However, you must visit a driver licensing office in person if you want to renew your CDL.
